Every reviewer performs an individual review to assess the quality of the work product and to identify anomalies, recommendations and questions by applying one or more review techniques, such as checklist-based or scenario-based reviewing, and logs all of them. The discussion of those items, and the decisions on their status, ownership and required actions, belong to the following activity.

Question 2

Before individual review begins, the review leader distributes the work product, explains the scope, objectives, process, roles, and work products, and answers participants' questions. How should this activity be interpreted?

During review initiation, the work product and other materials are distributed, the scope, objectives, process, roles, and work products are explained, and participants may ask questions. Planning defines those matters earlier; it does not distribute and explain them to start the review.

Question 3

In a review, each item the reviewers wrote down is now taken in turn: the group decides whether it is really a defect, who will handle it and what must be done, and then judges the quality level of the work product. Which review process activity is this?

Since the anomalies identified during a review are not necessarily defects, all of them need to be analyzed and discussed. For every anomaly a decision should be made on its status, ownership and required actions. This is typically done in a review meeting, during which the participants also decide the quality level of the reviewed work product and what follow-up actions are required.
